The warning icon that is displayed to the user when attempting to refresh or use the browser's back button does not have an accessible name, which means that a screen reader will not be able to ascertain the importance of the message.  

  • 1.3.1 Info and Relationships: Radio buttons not programmatically associated with the question  

When tabbing to the radio buttons with a screen reader, the associated radio button question is not announced. Additionally, a screen reader user cannot see the question when listing available form fields.

When tabbing to the check boxes with a screen reader, the associated question is not announced. Additionally, a screen reader user cannot see the question when listing available form fields.

'DUMMYSPOKE' being announced to screen reader users as they navigate through the page, just before the 'You must verify your identity (ID).' text. 

Screen reader users may be confused when then hear DUMMYSPOKE when completing the journey. Remove this wording.  

  • 1.3.1 Info and Relationships: Textbox has no accessible name or associated <label> 

Throughout FIRS textboxes lack an accessible name or associated <label>. 

For example, on Page 11 - Add user, the 'FIRS account number' has no accessible name or associated <label> element. It is currently announced to screen reader users based on the associated legend 'VERICALACCORD'. 

  • 1.3.1 Info and Relationships: Combobox has no accessible name or associated <label> 

Throughout FIRS comboboxes lack an accessible name or associated <label>. 

  • 1.3.1 Info and Relationships: File upload button has no accessible name or associated <label> 

On pages with file upload functionality the file upload button does not have an accessible name or associated <label> element. 

  • 1.3.1 Info and Relationships: Textbox has incorrect accessible name 

The 'What is the arrangement?' textbox has an incorrect label. The accessible name is currently "What Is Input" which doesn't match the purpose of the field. 

  • 1.4.3 Contrast (Minimum): Button does not meet minimum colour contrast requirements 

On mouse hover the 'Add or select a registrant' button text does not meet minimum colour contrast requirements against the background. Currently the contrast ration of the white text against the grey background is 1.39:1. 

  • 2.1.1 Keyboard: Interactive steps of the registration process are not keyboard accessible 

It is not possible to navigate to or interact with interactive link elements such as 'Check your eligibility to register' and 'Add or select a registrant', etc. on the Complete your registration page with a keyboard.  

People with motor and mobility impairments who cannot use the mouse and rely on a keyboard will not be able to progress from this page, preventing them from interacting with any of the other pages within FIRS.

  • 2.1.1 Keyboard: Checkboxes are not keyboard accessible 

It is not possible to navigate to or interact with checkboxes on the 'Which information do you want to remain private?' with a keyboard.   

People with motor and mobility impairments who cannot use the mouse and rely on a keyboard will not be able to progress from this page, preventing them from interacting with any of the subsequent pages within this user journey. 

  • 2.4.3 Focus Order: Focus automatically moved to first field on the page. 

Focus is automatically transferred to input fields upon page load.  

Automatically focusing a form control can confuse people using screen readers and people with cognitive impairments.  

  • 2.4.4 Link Purpose (In Context): Purpose of links cannot be determined from the link text alone 

Purpose of the links cannot be determined from the link text, or programmatic name.  

  • 2.5.3 Label in Name: Visible text does not match accessible name of button 

Currently the "Your registrations" button has an accessible name that does not match the visible text of the button, making it more difficult for a user of speech-to-text software to select it. 

When the visible text and accessible name don't match, speech-input users who attempt to use the visible text label as a means of navigation or selection (e.g., "Click your registrations") will be unsuccessful. 

  • 3.3.1 Error Identification: No error message when word count exceeds 1200 characters 

Currently a user can exceed the 1200-character limit and attempt to submit the form. No error messages are presented to the user, the page simply refuses to load the next step. 

  • 3.3.3 Error Suggestion: Accurate error descriptions are not provided 

Incorrect error descriptions and steps to fix the errors are provided to users, for example on page 4 - Do you have a UK passport? when inputting the passport expiry date. 

  • 4.1.2 Name, Role, Value: File upload button does not have an accessible name 

The file upload button does not have an accessible name. It is currently announced as required button FIRSVERIFY_UPLOAD_UPLOADFILE1 to a screen reader user. 

  • 4.1.2 Name, Role, Value: Menu button not correctly announced to screen reader users 

The expanded/collapsed value and associated instructions for the 'Account home' button are not being correctly announced by screen readers. 

  • 4.1.2 Name, Role, Value: Button incorrectly announced as unavailable 

Sections that allow the user to expand or collapse the contents are being announced as unavailable. On page 2 - FIRS Registration Process for example, the 'Registrant' button announced to screen reader users as 'button unavailable'.  

  • 4.1.3 Status Messages: Session timeout not in a live region 

The session timeout warning is not contained in a live region and is not automatically announced to screen reader users.  

Screen reader users who may not be able to see the screen may miss the timeout message. 

  • 4.1.3 Status Messages: File upload and deletion not communicated in a live region 

The successful upload or clearing of a file is not contained in a live region and is not automatically announced to screen reader users.  

Screen reader users who may not be able to see the screen may not know that the file has been successfully uploaded or clearer. 

We plan to fix these issues by July 2026.

We tested the service based on a user's ability to complete key journeys. All parts of the chosen journeys were tested, including documents. Journeys were chosen on a number of factors including usage statistics, risk assessments and subject matter.
